Attached is the consent and a list of what labs are offered and the price. Don't forget you must be fasting for 12 hours before your blood draw. Light snacks, coffee and juice to enjoy after. Bring the form with you or complete onsite. You can pay with cash or check, insurance reimbursement forms available.
